Waivers & Forms
Once you have created an online account with us, you must complete the waiver forms online in our website before your camper can attend their first day!
Follow this step-by-step guide to fill out the forms (applies to any of our programs).
Waivers STEP-BY-STEP GUIDE

Click the Enroll button.
Our Online Enrollment site will launch in a new page or tab.

Scroll to the bottom, and Sign In on the left.

Once you have accessed the system, scroll down until you see the View My Registrations section, and click the View Details button for the active registration.

Once the details page loads, scroll down until you see the Forms section on the right. This list displays all the forms and waivers available to be completed in order to finish the registration process.

Click on the name of the form to open it. Once the new page is launched, you will be instructed to fill out or initial the form. Items marked with a red asterisk (*) are required. Once finished, click the Complete this Form button. Repeat for all needed forms.
